- Pathophysiology
- Gluteus medius/minimus tendinopathy (degeneration/tear) at insertion.
- Note: True bursitis is rare; “Trochanteric Bursitis” is an outdated term.

- Clinical Presentation
- Lateral hip pain worsened by:
- Lying on the affected side (sleep disturbance).
- Weight-bearing/climbing stairs.
- PE: Point tenderness over Greater Trochanter.
- Key Differentiator: Hip range of motion (internal/external rotation) is preserves/pain-free (unlike OA).
- Differential Diagnosis
- Hip Osteoarthritis: Groin pain + ↓ Internal Rotation.
- Meralgia Paresthetica: Lateral thigh paresthesia/numbness only (sensory nerve compression).
- Management
- Dx: Clinical (X-ray rules out bony pathology).
- Tx: NSAIDs, Activity modification, Physical Therapy, Steroid injection (refractory).
